Employees who wish to make a change to their Federal taxes, must complete a new K-4 form. Keep in mind that in Summer 2018, The Commonwealth of KY instituted a 5% flat income tax for most working employees.
That form must be submitted to payroll for processing. To streamline this process:

- Scan it
- Create a HR Support Desk ticket
- Attach the K-4 scan
- HR will process your request for you
